POSITION SUMMARY
Under the supervision of a Registered Nurse, the LPN is responsible for planning, providing and evaluating care for
Hospice patients, assesses client and family physical, functional and psychosocial needs and facilitates a
treatment plan to meet those needs.
JOB REQUIREMENTS
1. Graduated from an accredited school of nursing.
2. Currently licensed as a Licensed Practical nurse with the Virginia State Board of Nursing.
3. At least one year generalized nursing experience.
4. Prefer one year experience with a home health agency or hospice working with terminally ill patients and their
families.
